    How to Order Custom Plates

    Step Description
    1 Download and fill out the Custom Plating Service Request Form (.xls file, 85 KB)
    2 E-mail the completed form to customplating@appliedbiosystems.com
    3 Your sales representative will contact you in approximately 2-3 days with a price quote and help you place your order.
    4 Your custom plates will arrive approximately 2-6 weeks after you place your order.
